Hello Urbana Families,
Online registration for returning students is open until July 16. You can register your student(s) through
Skyward Family Access. If you need help with online registration, please email
onlineregistration@usd116.org and include your student’s name and school.
In person registration
If you prefer to register your student in person, please mark your calendar for our registration event on July 30 and July 31 at Urbana Middle School.
- July 30 from 12 to 6 p.m.
- July 31 from 9 a.m. to 1 p.m.
This event is a one-stop shop. You can register your student, get required physicals or immunizations, meet school staff, pay fees, get food service information from Quest, talk to First Student/MTD about transportation, learn about after school care and more - all in one place.
Required Residency Documents
In order to register you will need your Photo ID and the following residency documents
One of the following:
- Mortgage documents: A copy of the the deed, closing documents, or other mortgage documentation which shows the parent/guardian’s name and address
- Rental lease with the guardian’s name and address
- OR -
Two of the following:
If you do not have mortgage documents or a lease, than you will need two of the following showing your address:
- Phone bill
- Recent utility bill from the last 60 days (gas, water, electric, trash)
- Public aid/Medical card, medical bills
- Current homeowner’s insurance or renter’s insurance policy statement
- Federal or state income tax return (financial information may be removed)
Required Physicals and Immunizations
Students entering kindergarten, 6th grade, and 9th grade must have a physical by the first day of school. Your student will not be allowed to start school without the completed health certificate. Students in 12th grade also have an immunization requirement. Please schedule your physicals and immunization appointments as soon as possible.
